Once you have an agreement with a buyer how do I actually "deliver" the domain to them? Do I initiate the transfer to them through my registrar or do they initiate it and I accept? I have all my domains registered privately so do I need to "unlock" them before the transfer? Thanks

You could ask the seller to create an account at the registrar(s) where your domain(s) are held. Then you can "push" the domain from your user account to theirs so they immediately gain control of the domain. They may want to transfer to their favorite registrar (if they have one) for consolidation purposes.
It is best to ask them and suggest that a push is more expedient.

You don't initiate a transfer, it's up to them. They do so through a registrar of their choice who will send an authorization request to you. Your domains must be unlocked first and you need to provide the buyer with the EPP codes for each domains. They will need them to initiate the transfer. You should normally be able to get the EPP codes by logging in into your registrar account.
This is the procedure for .com .net .org .us domains. For other TLDs the procedures may differ.

yes you need to unlock them and get the codes, just as sdsinc says. also be sure to receive payment before the transfer ;).

Yes -- that's very important. When I first started out, I'd be eager to push the domains and was scammed out of a couple. If it's for a large sale over $100, I'd recommend using an escrow service so you're not scammed out of your money somehow.
