Several new-to-this questions re: .com domain (re)registration

I bought a .com and hosting via a UK company last year; they've let me down big time; my domain/hosting package is up in early Feb.

I wish to transfer and renew my domain name to another provider. I also wish to have the hosting at another seperate company - easier maintenance (example: with my UK provider I couldn't even update WHOIS details myself - took 2 months and numerous communication to get it updated to my new married name). For the domain I'm looking at godaddy.com (or someone else as big and which also MUST offer privacy WHOIS protection).

Questions:
1a) Providers have a transfer fee (example: godaddy around $6/year). They also have a renewal fee. Does this mean that I will have to pay $6 to transfer my domain to them, and also pay $6 to renew the domain itself if I go ahead and push this forward before the Feb domain expiry?

1b) What happens if I try and transfer AFTER the early Feb expiry date - will I just go to a new provider and pay my $6 fee and they'll renew on my behalf?... or can this not be done? I.e. can I even move a domain after it has just expired with my current provider?

2) Does it matter about the name in the WHOIS records? My legal pre-married name has been kept on all my credit cards, and the married surname that I took from my wife is now on WHOIS and all my more "personal" records... will the new registrar care about this discrepancy as long as I can provide unlocking codes/telephone or written confirmation showing that I use the two surnames professionally and personally?

3) Any recommendations for US/UK based domain registrars with privacy, and hosts which don't require the two services together?

1b)If it is a name that would not be very well known i would just let it expire then reg it somewhere else.
