Help in filling W8BEN form (DomainSponsor)

I've some doubts on how filling correctly DSponsor W-8BEN Form, who may help me, please ?
I'm a foreigner individual (Italian).
I filled in Part 1 with my postal address and I identified myself as
"Individual" checking only this option.
I need to know: what's the reference number requested on point 8 ?
In Part 2 I should only specify that I'm a resident of "Italy" on point 9a
without checking any of the other options, correct ?
Now, what else should I fill in, among all the following points (apart
signing the form and entering the date of the day, of course) ?
What should I enter in the field "Capacity on which acting" ?
