Can SEs blacklist individual pages?

If this type of question has already been addressed, I apologize.

I once had my index.php page saturated with a keyword and as you assumed, the page didn't rank at all on Google. I optimized the page to slim the keyword density to about 4-5% for the keyword.

Now I'm wondering: Do SE's ever ban indivudual pages? Do they blacklist certain files if they believe they appear spammy? I'm not sure if I should leave my index.php as it currently is & allow time to heal the wound or if I should assume my page is useless & should be replaced with a default.php. Any ideas?

Hi,
As far as my knowledge goes, Google when bands, it bands the whole site. Nothing as such individual page or files.
