how do tld extentions get started? can anyone start one? there seems to be more domain extentions popping up all the time. if i wanted to start one like .med would it be possible?

No. In order to get a TLD you would need to meet criteria on multiple levels, both financial and technical. A lot of the media coverage seems to suggest that just about anyone who wants to can run their own domain extension—the reality is that they can't.

Can you imagine DNS system once anyone have custom tld?

It's very easy to start one's own TLD. Many organizations have their own (ie .local) but they will not resolve for the rest of the world.

For your TLD to resolve everywhere you need to convince icann to add it to the DNS root.

Lots of $$$ and connections, and yes you too can start, as in getting it accepted into the dns root, your own TLD for most anything. Crazy TLDs, such as .CAT ( .CAT ), all too well illustrate this.
