Toolbar page rank is a static snapshot of a number of factors that is updated every 90 days or so. It's a Google measurement and has nothing to do with other engine search results.
Google also maintains an internal PageRank which is constantly changing - there is no way to know that is, or the "secret sauce" which produces it (unless you're a G. engineer) and there's controversy as to whether what is shown in the toolbar includes ALL the factors used in their internal ranking system.
Interlinking (number, relevance and quality of links to and from the page) plays a huge role in determining toolbar page rank - I've seen new pages with just one strong, mid-PR inbound link pick up similar page rank on the first update (i.e. one PR6 link producing a PR5 page). This is of course assuming there are no conflicting elements to devalue that page (keyword spam, linking to "bad neighborhoods", etc.)...
SERPS placements involve many factors. Since TBPR is a static value, its impossible to have it be a constant influence across a variety of searches since any given page is more relevant for some querys than for others. The page could be #1 for one term and number 30,001 for another.
If you look at actual search results for a bunch of keywords, they're very unlikely to be in (toolbar) pagerank order.

How new is your site? Are there links to it? It can take a while for new sites to show up in G. and Yahoo, MSN is faster. Aside from that, reasons for not getting indexed vary. If you're using a domain which was ever associated with spam or blacklisted for other reasons you may have to submit a re-inclusion request - even if a previous domain owner was the culprit (always check the history before buying expired domains!).
