I think most are one time fees. I know in Ohio, you have to pay about $25 every 5 years to keep your business name and standing active with a "Statement of continued existence" filing.

Most states have a website that contains all the information you could wish for about forming and running a business. Often times they even have online applications or forms for filing for a business name, LLC, etc...

http://business.illinois.gov/ Might want to start there.

Don't forget additional costs of filing annual reports, tax returns, etc. for entities.

LLC fees are likely to be yearly in addition to the one-time fees, as states tend to follow the "leaders" on LLC's, Nevada and Delaware.

The annual fee for a Delaware Corporation is $60.00 or $200.00 for an LLC, and a Nevada Annual Corporation or LLC fee is $85.00 for either. While these are not standard (Obviously), you'll need to look into the other costs that go into an LLC other than the one time fees or legal expenses you incur.

As noted above in Nevada, the simplicity of an LLC is balanced against its heftier tax/fee schedule. As another example, North Carolina is $200/year for an LLC ("Annual Report Fee"), but only $15 for C-Corps.
