How do you determine the unique visitors through the keyword tool, by typing the keyword or keyword + .com, is the global monthly searches the monthly unique visitors per month?

You can't. It has no information about your domain/web site.

If Google Adwords Keyword is telling you 1000 exact match searches..

And you managed to optimize the keyword for first position then you would get 50% visitors of 1000 exact match searches...per month.

keyword + .com will be the fraction of website visitors how have entered keyword.com into google search or the browser address bar (at least with some browsers) so it was searched by google before they we redirected to the website. So that corresponds to a part of type-in visits to website, but not the visits via backlinks, content search results, ads, and other means.
