yes this is fairly simple if you have PHP enabled on your hosting server.

you should browse through the programming section of this site, i am sure you will find what you are looking for there.

Is not so simple for me. The problem is that I don't have PHP enabled on my hosting server or any scripts, but I 've found some sites that offer free forms and they also host it, like formmail or www.emailmeform.com . Thank you again for your suggestion.

What is your host? If you have paid for hosting and they dont provide PHP, that is pretty bad.

Do you have CGI?? Maybe you can use a perl form??
