$75 Easy Wordpress form needed. (see Attached):hi:
Requirement:
We are looking to integrate a form into our Wordpress based site. This form must save the input fields to the database as well as send an email notification to a list of site users that the form was filled out.
The information then should be viewable in the admin panel and downloadable as a CSV and/or XLS file.
Form fields are as follows:
- 5-10 Text fields
- 2-3 Textareas
- 1 file upload (This will be a picture, it should not be stored in the database but rather as a file)
The plugin we would prefer to use is the Ninja Forms plugin by WP Ninjas (http://ninjaforms.com/). This should have all the functionality we need but it needs to be integrated into our existing site with custom CSS.:wave:
