1. Add 3 Fields in Mysql

a.Phone
b.Comments
c.other


Then add those three fields to forms on 4 Php pages (I already have the form with other info ..these 3 fields would be additions)



The visitors fills out the form and the information is displayed on pages in spreed sheet format... I already have the spread sheets and that display 8 fields per column ..because of that there is no room left to display the results of Phone, Comments and other..so we will use a simple mouseover/clickover to display that info in the spreesheet

Pretty basic, 3 fields in Mysql, add the new fileds to the forms and diplay the info on the pages
