I'll pay $25 to code up a contact form with a php file

Hi,

If anyones interested in helping me I need someone to code a simple contact form, with name, address, email, and message and have a php form that sends info to my email address.

The site was done in tables (yes I know bad). The page is design is already in table form I just need the contact form to look a certain way, design wise and it's taking me too long to nut out. I'm sure it would done with css.
The form part will need to look like this

I already have one php form that I used for the flash site, so you can use that if you want.

I actually don't think it was as easy as you would think!

I had a few people say that it would be easy and tested out a few people and they were finding it hard.

That little square form was to be embedded in a web page, the page was done with tables and that's where it got sticky, especially in cross browsers.

Mr Internets did very well and stuck exactly to the design, colours, fonts and alignment to a tee!
