Help with a DB problem! Make field entries go to .pdf file!!!

need some help!! i need to know how to make the inputed fields in a form, when submitted, go to the corresponding fields on a .pdf file. have acrobat, and can't figure this out. .pdf needs to be printed after all info is completed! any help would be great, pm me for more details if need be.
