Ever lost a domain due to a registrar mistake?

Have any of you ever lost a domain name due to a renewal mistake by your registrar?

I stumbled upon this article where James Skene lost his domain:

I'm not sure what to think about the story above, but it made me wonder how often this happens, (and inspired me to start renewing as soon as I get my 90 days notice).

Frankly, it sounds like the author of this article is a clueless whiner writing a lot of assumptions he clearly hasn't taken the time to look up.
