Easy database job

hey guys,
i need a database created with the information from a small form, (which is also my aweber form (www.godteens.com/thestand.html).

Just the peoples name and age would then be projected onto my website, (I want the names and age to scroll down inside the white box on the side of my site.

I also want the total number of people who have joined to be on my site, updated as people register. (as displayed on the PSD below.

it is for this site. (template only at the moment)
