TR system broken?

I can't leave TR for someone:

The feedback works, but it gives the database error which usually makes people refresh and leave it in duplicate. I haven't found what is causing this yet.

OOps, i seem to have left TR in triplicate as well. If someone gets a chance, it'd be throughly appreciated if that is corrected. Thanks!
