I just got done coding this and I been wanting to do it for weeks now. There is a lot of spam happening with contact forms lately. I edited my form to prevent BCC usage but still the bots make the form attempt. So I decided to implement image validation as well.
The setup is great. The contact form is a single php page. It call an include folder which has a config file, fonts and the image generation file. The contact form executes as a php_self. All code is rather easy to change and understand. I can do custom contact forms too if you wish (extra cost PM for quote) to add things like a dropdown subject list or drop down mailto list. It requires only php with GD lib (all common). No MySQL is needed or special CHMODs. Very easy to install and setup. Form is also tableless. You can easily make html changes to this that will suit your needs.
I am selling this at $10 a pop. For $15 I can also install it for you on 1 site. The rights DO NOT INCLUDE RESALE RIGHTS but one purchase will allow you multiple installs on domains you may own.
