simple and gets the job done... not much more to say.

I do like the fact that the address field is quite large and the first thing visible pretty much.

nice design but i have noticed bigger text fields = less revenue as the people do not spend time searching for the text field

therefore it depends on what ur ultimate objective is

In terms of simplicity, you've just won 1st. place. Nicely done. Even someone that's never visited a proxy before can navigate yours and doesn't get lost in an avalanche of google ads.
For a proxy, very nice work!
