Is it just me or is your domain is redirecting to another domain?

I have had problems like this too with new sites, but after some time they start to show up (don't know why)

How long has this been happening for?

For those having 2 out of 3 ads not show, that is usually meaning that there are no ads to show on those other ads, of which Google then serves a blank where they should be. In your ad configuration, you can set a URL of your own ad to fill in these spots. You cannot, however, use another contextual ad system like fastclick or yahoo or whatever. I use the alternate ads on one site to show Netflix ads from LinkShare.

Sometimes it is just a matter of "fine tunning" the keywords on the site. Once optimized, the ads should appear in every block instantaneously.
