Javascript Developer needed for quick form. $10

Hi all

I need a javascript person to whip me up a quick form for which i will pay $10.
I've already coded the HTML, just need the dynamic stuff added in.

There are only a couple of fields that you'd have to code for, so it shouldn't take too long.
