Form Fields are where data is stored in the database. For every piece of data you plan on getting from your leads, you must have a form field. To access the View Form Fields page, where all of your form fields will be created and edited, click the Form Fields link under the Settings tab.
Click the Add a New Form Field Button to create a new field
* Field Type
o Checkbox
Checkboxes give the user options.. Multiple choices are possible with checkboxes..
o Drop Down List
Drop down lists are pre-populated with a list of choices.
o Input Box
Input boxes are simple, one-lined input fields for text.
o Multi-Line Text Box
This is an input box where multiple lines (a paragraph or two) of text can be added.
o Multiple Select Box
Hold the ctrl key to select multiple options from a drop down list..
o Radio Buttons
Multiple options are available for selection, but only one can be selected.
* Field Name
Enter the name of the field as you would have it appear on the form.
* Field Description
A description for the field also appears on the form, next to the field name. This is generally used to give the visitor more information about what should be in the field (i.e. date formats, phone numbers, etc.).
* Values
Values are the options listed in drop down list menus, check boxes, radio buttons and multiple select boxes. Enter each value one per line.
* Style
CSS values can be used to change the size, color, and appearance of your fields.
* Custom Validation
Use regular expressions to create validation for fields.
* Required/Not Required
Required fields must have information before a form can be submitted. Any field that has been designated ‘Not Required’ can be left blank in a submitted form.
* Published/Not Published
Published fields appear on your web form. Fields Not Published display internally, but not on the website.
