Favicon fix

i am movin on to fix my first favicon for my test and hope get a quick fix tutorial here.

1) what is the html codes for the favicon?
2) what is the standard size of the fav?
3) how and what do i use to create it?
4) on which file/folder should i paste it in?
5) do i pit the fav image in another folder?
