HSV-2 (herpes virus type 2) is the virus that causes most cases of genital herpes. About 25 percent of Americans have been infected with the virus. The symptoms can be subtle, so the majority of people do not know they are infected, and can pass the infection on to others. However, the disease can be disabling for those who develop recurrent, painful, genital ulcerations. Antiviral medications can decrease the risk of recurrent disease, but there is no cure for the virus. Condoms can decrease your risk of developing genital herpes, but they are not 100 percent effective. For these reasons, a vaccine against HSV-2 would be extremely useful, and extensive efforts have been invested in this attempt for the past 50 years. However, the development of an effective vaccine has been problematic. A lot of the difficulty is related to the highly complicated life cycle of the virus, and the mechanisms relating to recurrent disease. Though some types of vaccinations have appeared to work in animals, they have not been effective when studied in humans.

One vaccine has been shown to be effective in decreasing the risk of developing genital herpes for women, but only among those who have not been infected with either HSV-2 or HSV-1 (the virus that causes oral cold sores). For uninfected women, the vaccine was partially protective against infection (the risk of infection was decreased by 40 percent), but it decreased the risk of developing symptoms of the disease by about 75 percent. However, since many sexually active women have been already infected with HSV-2, and about 65 percent have been infected with HSV-1, most women will probably not benefit from this vaccine. For unclear reasons, the vaccine does not appear to be effective for any men. The Herpevac Trial is currently underway to further study the effects of this vaccine. If the vaccine is proven to be effective in further studies, it could potentially become commercially available within the next few years. Other vaccines against HSV-2 are also under development, but several years of further study will be necessary before they will be available for clinical use.
