I believe its the advertiser that pays this amount as those doctors or lawyers get quadruple amount for leads that signs up either to retain their services or for treatment.

i dont think that there is any parking company involved when u are dealing directly with google and publishing ur ads on ur own websites

Actually this is the MAX cost a user pays for the keyword - with the qual. score algorithm, they end up paying much much less, especially on content and parked sites, if they even chose to run on the content network, that is.
