Customer handling script

I'm seeking for Custom handling script . What i need is users to be able to submit a ticket for a service request and able to track their ticket if their order is done or not. I know it's similar to Support scripts but i need customs forms and not stable Name, Email, Subject.

You can setup custom fields if you would like - perhaps that route would work for you. I'm not certain if some of the other scripts out there would do it as well.
