I need a Widget built for wordpress, that should be simple to install for any wordpress user.
It will be a 6 question survey, with results at the end based on the inputs and of course a link back to our site. Needs to look nice, and you will need to create some graphics to go along with it.
Rules:
1. PM with Price Quote or questions
2. No payment will be made until the item is working, error free (my reputation should insure I will pay).
