Do any of the registrars offer whois privacy included with the domain registration or is it pretty much an extra fee wherever you register? If it is always extra which registrars offer this service for the cheapest?

Register Matrix is free and so is NetFirms but I've never used either one of those so I can't vouch for them

or, "do it yourself privacy", a PO Box and voicemail number to use in your WHOIS info which shields your name, home address, phone number yet still allows the registrar several ways to contact you which keeps you ICANN-compliant:

First Name: <domain, whois, dns, etc>
Last Name: <manager, admin, contact, etc>
Address: <PO Box>
Phone: <voicemail #>

You can get a free voicemail # from ringcentral(d0t)com. If in US, add the voicemail number right away to the do not call list @ donotcall(d0t)gov

Thanks Fitz, very helpful, rep given. Have you used Moniker? Also, with Go Daddy is that something you have to ask for or is it given automatically each time you register 5 or more domains at a time?

You can do it in your shopping cart, once you have 5 names, select privacy and it should add it without charging you any money.
