Random Picture Survey Help

I need to display a Random picture and enable the user to rate this person using radial buttons (on a scale of 1-7 or strongly agree - strongly disagree) and have that information ( which picture and what ratings they gave them) recorded so it can be possessed


My skills in HTML code isn’t' strong and I don’t think Frontpage is up to the task. Can anyone HELP me? Any suggestions about the best medium to go about this?
