Need a Shopping Cart with Custom Template

I need a shopping cart setted up right on my website with a custom template.

I need something similar to

Who ever i hire will also perform other task throughout the year if first transaction goes well.

I am looking to spend aprox 100 canadian dollars

Script can be Oscommerce, Znet, etc. I prefer Oscommerce but any good script will do.
