Mailing list Form - best method?

Hi, what's the best way to add a mailing list form box to your site? So far I've been using a free Bravenet one, but I'd like to be able to customize the way it looks and what it says.

Is there a basic HTML template for that? And if so, how would I set it so the form info is still sent to my bravenet mailing list?

You will need a server side scripting, probably a PHP mailing script. HTML can't handle this if you mean "mailing list".
