Looking for E-mail form with drop-down contacts, required fields...

Hi,

I'm looking for a free, secured customizable and reliable em-mail form that will allow the users to choose the contact e-mails from a drop-down list, that will have "Name" and "E-mail address" as required field and of course a message and subject field.
I haven't got any knowledge in PHP, so an script that's easy to install would be perfect.

Any suggestions?
