The script is off the shelf (Phurl 1.1, GPL licensed) but I have spent hours editing the HTML & PHP code by hand to conform to .MOBI guidelines and cater to mobile (phone & PDA) applications.
Front page scores 5/5 on Ready.mobi & valid XHTML Mobile Profile.
Simple password-protected admin interface (delete shortened urls)
