I need Simple Contact Form

Hi guys,

Im looking for somebody to build me very simple contact form, soon as possible (i'll pay of course, just say how much & 1st person with reasonable price gets it)

must be in html or php

like contact form on:

Dignity Home Health: Employment Opportunities

But instead of a pull down box next to 'location '
Just make it a text area where user can type their location

Dont need the 'position' line

And keep the 'resume upload' just like it is on the example site

Should be easy enough

Do you have gmail- you can make easy webforms with google now! Just go into your gmail account and it should be on a menu to the left. It's free. You can embed it into your website.
