PlayTouchGames.com (Note: Averages in title based on Jan-Jul '08)
I started this site just for playing games on my own iPod Touch, but decided to post some links around and it became an instant success. The first full month, my revenues were nearly $200, the second month over $300, and the third month well over $800 and they've been right in that ballpark ever since. Last month (July) was my best month to date with $854 revenue. All revenue is from AdSense ads. The majority of traffic (70% or so) is organic, with a good majority of that being from Google, with "ipod touch games" as my best performing keyword. CPM the past couple months has been roughly $3. The price is negotiable. I've enjoyed sitting back on the monthly revenue, but I could really use the money from a sale.
There is no work that needs to be done. Everything on this site is set up. You can always add more games or apps, but it doesn't need to be done. There are 36 very popular games optimized to work with the iPod Touch and iPhone. Adding games does bring back return visitors, however. I haven't done this in almost 2 months or so.
Looking for $15,000-$20,000 range offers. I know this is on the high end, but I know it's worth it. The iPod Touch/iPhone are not going anywhere any time soon, and the best time of the year for this is coming up - revenues will be higher and higher as Christmas nears. The site has always had awesome CTR's (2-3%) and very good CPMs.

Costs are minimal. I'm using GoDaddy shared hosting with several other sites of mine, so I'd say maybe $5 per month.
