E-mail form

I need to find a script like the one used on duiprocess . com, the one that automatically sends a FREE report to you. I'm a n00b in PHP, so I'll need a hand here I'll take a look at hotscripts . com later on today. It has to have a name field, e-mail field, and once a visitors enters them and presses Submit an e-mail is dispathed to him with "Dear Name," on top. Name and e-mail are added to a .dat file on the server.


If anybody could write me that script I'll pay 300 np$. Easy job. :tu: I don't care if you modify an already made script, but I need everything just as described above (besides .dat, that could be any data extension)
