I made a proxy design for myself and ended up not needing it.

The proxy is fully coded in HTML & CSS. I has the script PHProxy, that is widely used among proxies. I have a preview posted that is the fully-working template.

I learned css at w3schools.com. It's a great place to learn stuff, they teach you valid code and everything.
