$10 to Fix - AJAX JQuery Form Validation

I was trying to apply the JQUERY concept explained at tutorial this

Code is attached...

For some reason, form validation is not working! It should act as the validation in this form.

I'm sure it's a small thing I'm missing, but I don't wanna waste time on it.

If you can fix this and make it work for $10, please post here before you do the job. I will then post back saying that I agree.

Job has to be done in max 2 hours from the okay time.

You need to send me back a zipped file with the functioning code.
