ICANN unfortunately has no control over Nominet. They are the sole guardian of UK domains and act at their own discretion. If you don't have any address proof in the UK, you can ask anyone there (perhaps any domainer in namepros may come to your rescue if asked for). Alternatively their are...

poll Dot US vs Dot USA, Will It Actually Happen?

On the domainer's perspective I would prefer .us over .usa. What I am waiting to see is how the .usa pricing model works. If they declare all common keywords as "registry premium" leaving hardly any place for domainers to invest, then I will be the least interested.
