I am looking for an appropriately skilled person to design a form that collects required information from a visitor to the web page and then emails it to a designated email address in a readable format.
To be able to do this project you will need to have graphic design skills as well as some coding skills. My primary concern is that the form is secure and any code/script behind the form is protected so it can not be abused. The design/code must be compatible with commonly used browsers today.
The form will have around 12 fields/options on it and will require some very basic data validation (eg, ensuring a field has data in it before submitting).
To be eligible to do this work you will need to have a portfolio of/or be able to demonstrate past work done of a similar nature.
