Urgent PHP help required.

Hello.

We have a web page in html that need to be rewritten in PHP.
This is the page:

It needs to be in php so our affiliate software will work, as it doesn't seem to be working in html.

PHP code needs to be added into each of the Options 1 to 9 so a button shows for PayPal so people can pay and get credited to the affiliate.

Is anyone up for it?
Please post here with you price if you are interested, we need this doing real quick.
It is only this one page that needs doing.
