If I have a template in a zip file that contains PSD file and an HTML file. If I modify the PSD file do I have to re-slice it and re-code it? OR do I have to go though each image in the HTML images folder and modify bit by bit for the design? Does this make sense?

It depends on how much customization you want to do. If it's only minor, say the logo or something you could simply do the alterations, find out the exact size of the old logo image and cut the new one to the same size. If you need to be more precise you would have to put the old logo over the new one, line it up exactly then pull in the guides to get an exact fit.

I'd have to change all the images and text within the images. I was hoping that the slicing would stay intact, but I guess I'm back to square one...

I agree with what is said above.

If you make minor modifications you dont need to re-slice it.

Anyway I would suggest that you re-slice it and code it by hand - we all know that templates are very bad coded .. and with no attention.
