Polls/Surveys + Feedback software?

Hi all.

I'm about to do a beta-test of a video game that I'm working on, and would like to know if there's any website software out there that can let me easily setup a survey/poll that beta-testers can use to give feedback on the game?

An example of what it would need to support:

Do you like X weapon?
[ ] Yes
[ ] No

Please write your comments on X weapon in the box:

...and there would have to be MANY of these (for most of the game's larger features), possibly spanned across multiple pages.

Is there some software that allows me to set that up? I'm not a programmer, so something automated would help a great deal. (otherwise, I can get one of my programmers to figure it out)

I could throw something together exactly like that, and it would send the results to your email. Not too hard to change whatever you needed either. Let me know if this is what you want.

Thanks a lot for the offer. Using that idea (something that goes to your email), I was able to setup something similar on my website (following a FrontPage 2003 tutorial I found), but I realized that what I really need is averaged-out stats (like the polldaddy polls) so that I can see how many people have voted for something, rather than trying to figure it out by reading hundreds of emails manually.

In other words, rather than getting individual emails that say what each person voted for, I also need to see a graph that says, "90% voted for A," "10% voted for B." That kind of thing. ...because there are going to be TONS of options, and TONS of beta testers. Too many to filter through each thing manually.
