Im looking for someone competent in PHP/ MySQL to make a form and database for my photomodelling website. I am taking care of the design aspect so it is the functionality of the form and the intitial setup of the database and tables within the PHP admin/ My SQL server so that i am able to update and manage the site. I need an application form with parameters (x) very similar to http://www.glamourgirlphotography.com/modelapplication.asp and member area that has secure login and password that is able to generate new customers and automated emails and the ability to upload photos for me to assess and put on the site.
