Finding the right end-user is key here.. high initial reg cost too! I'd say to the right MD, low x,xxx is possible. I suspect the variations on this for .com, etc. were already taken?

If i was a doc, i think id prefer the easier name and the ext that doctors are going with .md Nephrologist.md sounds better than Nephrologistmd.net, its also easier to remember.

Also im going to list it for Sale on Ebay, Afternic,Sedo,Pool, and Great domains, where do you recommend selling ?

Hm, in this case, you "might" have a better chance of top dollar on ebay.. because most domainers are resistant to the ccTLD's... I'd also include a small site with optimized content of some sort if I were you.

Regular folks won't know what the heck that is and most certainly won't know how to spell it.

You would need to appeal to those in the know - a very specialized area.
