Need help programming Flash contact form with PHP

Hello,

I have the PHP coding to program a basic Flash form - however, I'm trying to program this form (the site is still in progress, excuse the bumpiness) and it incorporates drop-down menus and I'm not sure how to do that.

I need to find someone who knows how to do this, who can edit the current PHP form I have set up, and can explain to me how to do this so I can do it in the future. I will pay you for your time and knowledge. Please PM me/post here if you can help and let me know what you charge.

That only covers the basics that I know how to do - what I'm having issues with is how to program the drop-down boxes.
