Need e-mail form and Nav bar coded ASAP

Hi guys, thanks for viewing. Simple work request here, we just need a basic contact/e-mail form to be coded for our 'contact us' page. Just the basic Name, e-mail, and message fields for easy mail submissions to our e-mail account. Also, our navigation bar needs to be recoded and placed properly within our header.
