Need a simple PSD converted and a contact form.

I need a very simple PSD to be converted.

If the same person can do it. I need 2 forms to be created and the data be forwarded via email. If possible to add a captcha.

I have uploaded the png. The forms must also be with the same background.

Can I do it in WordPress using my genuine developer's license for Gravity Forms?
