Looking for a script(s)

Hello!

I am looking for a script(s) that does the following:
- shows the number of views
- shows the number of comments
- shows the number of ratings and the "stars" that rate the script
- shows the user's comment
It has to be similar to what you see on youtube.com . It has to use PHP and MySQL to work (no CGI scripts)
I want to incorporate this script in my new video website so I don't want a complete script like a youtube clone... just the things I requested.
It has to be free

Did any of you use such a script(s)?
