Is it possible to make a php questionnaire?

I am planning on putting a quesionnaire together to ask 200 people about a new product / service I am starting. I have considered all of the options of paper, paying for a website service etc.

However, I have built forms for my website, eg, email contact forms with some buttons and fields to fill in. It then submits it to my email account automatically with the answers.

What are you thoughts on whether this works for a questionnaire of about 30/40 questions which I want to send to 30-40 people.
They can fill in their names and details at the top, just as a paper copy would work.

Any good and bad points would be very much appreciated.

Thats not a bad idea.
As long as you know the people who are filling it out.
I would not give this link to those you do know know or trust, as forms are very easly manipulated to hack your site if they are not done correctly.

But other than that, give it a go!

30-40 questions sound a bit long to me. People have busy lives in the 21st century.

I tend to agree on this. 30-40 questions are a bit too many. It would be a tough ask for you to convince people to fill up those. I think 15-20 questions will be good enough.

:imho: nowadays surveys comes with prizes built in like for example you get a bonus cd for every X number of surveys filled up even with a generous freebie attached to it people nowadays will just frowned about it, they kinda lazy.

but if the prizes/or freebie is kinda expensive might be possible.

Well this will be a more targetted survey at people who have used the specific site. What I need to know is what people think about using a php form for the questionnaire. I think this could be a good way of collating the data without having to use a template survey company which looks rubbish and costs loads.

I don't see any issues with people filling them in on a php form - does anyone else?

Since you are offering a new product/service perhaps with such a large questionnaire you could offer a discount to those completing the survey as well then offer a free product to a randomly drawn name from those who do complete the survey?

If you are targeting previous customers with the survey I would certainly offer them a discount on their next purchase/job order.
