Do you already have the skin? If so, it depends on what software you're using...

If you're asking how to actually create your skin, well, that too depends on the software, but a good place to start would be by taking a look at other skins and picking apart the code/images to see how they work. You can get certain tips on skinning, but we need to know what software you're using first...and you won't find a tutorial that will tell you everything you need to know about how to create your own skin

It would help if you told us what forum software you are using and in what format is the custom theme, is it coded? is it a psd?

That is a very general question.
What software are you using?
Is it a template for a forum you downloaded?
No one can answer questions just like you have very easily. We need details.

I would agree with everyone else, we need to know the specifics. In most cases it is as easy as uploading the files and checking which you want.

Well, an easy way is to just change all the images and replace them with your own skin images and then change the templates to have the look and feel of your image match with the forum.
